Thank you for your interest in the North Metro Blazers Homeschool Basketball Program! Once you have read through the program policies and codes of conduct listed on our website, please complete and submit the following form. We will contact you as soon as we are able. If accepted into the program, we will give you an access code you can use to access the Player Registration Forms.
